MENDON — Police responded to a fatal crash in Mendon today.
Authorities were notified of a two-vehicle head-on crash on US Route 4, near Woodward Road, at around 10:20 a.m.
Police say both occupants of the first vehicle were pronounced deceased by members of Regional Ambulance Service.
Two occupants of the second vehicle were extricated from the vehicle with life-threatening injuries.
According to the report, John Panoushek, 79, of South Woodstock, was traveling west on US Route 4 when he crossed the center line striking Beverly Adams, 73, of Stockbridge, head-on.
Police say the circumstances of the crash are still under investigation.
